TO OUR KNOWLEDGE, THE ADVERSE EFFECTS OF BURN PIT EXPOSURE HAVE NOT BEEN INVESTIGATED AT THE SUBCELLULAR LEVEL BUT COULD BE THE BASIS FOR SERIOUS PROBLEMS. THE IDEA THAT TOXIC EXPOSURE CAUSES BIOCHEMICAL AND EPIGENETIC CHANGES THAT LEAD TO SERIOUS CHRONIC HEALTH PROBLEMS IS SUPPORTED BY THE LITERATURE AND OUR PRELIMINARY DATA. WE COMPARED THE BLOOD FROM FIREFIGHTERS TO CONTROLS AND FOUND REGIONS WITH DIFFERENTIAL DNA METHYLATION IN GENES THAT REGULATE REPRODUCTIVE FUNCTION. SUCH EFFECTS WERE EVIDENT IN ABNORMAL REPRODUCTIVE HORMONE LEVELS FOR THE FIREFIGHTERS. SIMILARLY, WE PROPOSE THAT PARTICIPANTS OF THE BURN PIT REGISTRY POSSESS LASTING EPIGENETIC AND HEALTH CONSEQUENCES CAUSED BY THE EXPOSURE THAT REQUIRES INVESTIGATION.$2,020,950

**AWARDS ISSUED PRIOR TO JANUARY 20, 2025, WERE FUNDED UNDER PREVIOUS ADMINISTRATIONS AND MAY NOT REFLECT THE PRIORITIES AND POLICIES OF THE CURRENT ADMINISTRATION.** FOOD INSECURITY IS STRONGLY ASSOCIATED WITH INCREASED PREVALENCE OF TYPE 2 DIABETES (T2D) IN U.S. ADULTS. T2D REPRESENTS A PARTICULAR CHALLENGE TO LOW-INCOME FOOD INSECURE PEOPLE, WHO OFTEN LACK RESOURCES TO MANAGE THIS DIET-SENSITIVE CHRONIC DISEASE. EFFORTS TO IMPROVE THE FOOD SECURITY AND NUTRITIONAL HEALTH OUTCOMES OF INDIVIDUALS WITH T2D MUST ENGAGE THE PROBLEM AT MULTIPLE LEVELS. THE DELIVERING HEALTH PROJECT INTEGRATES INNOVATIVE RESEARCH, EXTENSION, AND EDUCATION ACTIVITIES IN ARKANSAS, A HIGH-NEED STATE, LEVERAGING RESOURCES OF A MEDICAL SCHOOL (UNIVERSITY OF ARKANSAS FOR MEDICAL SCIENCES) AND ARKANSAS'S COOPERATIVE EXTENSION SERVICE. GUIDED BY STAKEHOLDER INPUT, THIS PROJECT HAS THREE OBJECTIVES AIMED AT IMPROVING THE NUTRITIONAL HEALTH AND SELF-SUFFICIENCY OF LOW-INCOME FOOD INSECURE PEOPLE WITH T2D: 1. IMPLEMENT AND EVALUATE AN INTERVENTION USING HOME-DELIVERY OF T2D-APPROPRIATE FOOD BOXES THAT INCLUDE PLAIN LANGUAGE ADAPTED EDUCATION MATERIALS AIMED AT IMPROVING DIETQUALITY, PHYSICAL ACTIVITY, AND HEALTH OUTCOMES OF LOW-INCOME FOOD INSECURE PEOPLE WITH T2D. 2. IMPLEMENT, DISSEMINATE, AND EVALUATE EXTENSION MATERIALS--INCLUDING A TOOLKIT AND CURRICULUM--TO ENGAGE FOOD PANTRIES IN SUPPORTING THE HEALTH OF THEIR CLIENTS WITH T2D. 3. IMPLEMENT AND EVALUATE AN EDUCATION MODULE TO IMPROVE GRADUATE MEDICAL, NURSING, AND PHARMACY STUDENTS' READINESS TO SUPPORT THE HEALTH OF FOOD INSECURE INDIVIDUALS AT RISK FOR T2D AND OTHER DIET-SENSITIVE CHRONIC DISEASES. THESE OBJECTIVES ARE BASED ON TWO USDA NIFA PROGRAM AREA PRIORITIES: WE EXPECT TO 1) IMPROVE FOOD SECURITY AND NUTRITIONAL HEALTH OUTCOMES FOR LOW-INCOME PEOPLE THROUGH EVIDENCE-BASED APPROACHES TO HEALTHY EATING AND ACTIVE LIVING LIFESTYLE PROGRAMS AND 2) IMPLEMENT AND EVALUATE INNOVATIVE STRATEGIES TO IMPROVE EATING PATTERNS THAT PREVENT CHRONIC DISEASE.$991,099

THIS PROJECT WILL USE MALE AND FEMALE TRP53?P/?P MICE OBTAINED FROM AN IN-HOUSE BREEDING COLONY AT THE UNIVERSITY OF ARKANSAS FOR MEDICAL SCIENCES (UAMS) WITH TRP53?P/?P BREEDERS PROVIDED BY DR. BALMAIN (UCSF). A TOTAL OF 366 TRP53?P/?P MICE WILL BE GENERATED AND USED THE TWO SPECIFIC AIMS AS FOLLOWS: SPECIFIC AIM 1: DETERMINE THE EFFICACY OF ?-TOCOTRIENOL AS A COUNTERMEASURE AGAINST MIXED CHARGED PARTICLE RADIATION-INDUCED CARCINOGENESIS. AT THE AGE OF 4 MONTHS A TOTAL OF 210 MICE WILL BE TRANSPORTED TO BNL. MICE WILL BE EXPOSED TO SIMULATED GCR (5 ONCE-A-DAY FRACTIONS OF 0.15 GY FOR 5 CONSECUTIVE DAYS) OR SHAM-IRRADIATION AT NSRL. ?-TOCOTRIENOL WILL BE OBTAINED FROM A NATURAL SOURCE AS ROUTINELY PURIFIED BY GUANGRONG ZHENG PHD (UNIVERSITY OF FLORIDA). MICE WILL BE ADMINISTERED ?-TOCOTRIENOL ONCE DAILY BY SUBCUTANEOUS INJECTION (50 MG/KG BODYWEIGHT) 24 HOURS BEFORE EACH RADIATION FRACTION. AFTER ?- TOCOTRIENOL ADMINISTRATION AND RADIATION ARE COMPLETE THE MICE WILL BE TRANSPORTED TO COLORADO STATE UNIVERSITY (CSU) FOR 18-MONTH FOLLOW-UP. MICE WILL BE EXAMINED DAILY FOR THE PRESENCE OF TUMORS USING ROUTINE PROTOCOLS IN THE LABORATORY OF MICHAEL WEIL PHD. MICE WILL BE HUMANELY EUTHANIZED IF PALPABLE TUMORS ARE PRESENT OR THE ANIMALS ARE MORIBUND BASED ON PRE-DETERMINED CRITERIA. ALL OTHER MICE WILL BE EUTHANIZED AT 18 MONTHS AFTER IRRADIATION. ANIMALS WILL BE NECROPSIED UPON EUTHANASIA AND ORGAN SITE AND NUMBER OF TUMORS RECORDED. OVERALL TUMOR BURDEN AND TIME TO OCCURRENCE WILL BE ANALYZED. SAMPLES OF TUMORS WILL BE COLLECTED FOR HISTOPATHOLOGICAL VERIFICATION BY A VETERINARY PATHOLOGIST WHO ROUTINELY EXAMINES TUMOR SAMPLES IN STUDIES ON SPACE RADIATION CARCINOGENESIS AT CSU. AN ADDITIONAL GROUP OF 60 TRP53?P/?P MICE WILL BE TRANSPORTED TO BNL AND EXPOSED TO RADIATION AT NSRL AS DESCRIBED ABOVE. THESE MICE WILL BE USED TO OBTAIN PLASMA SAMPLES AT 4 8 AND 24 HOURS AFTER ?-TOCOTRIENOL ADMINISTRATION ON DAYS 1 3 AND 5 OF IRRADIATION. SAMPLES WILL BE USED TO MEASURE CONCENTRATIONS OF ?-TOCOTRIENOL WITH LIQUID CHROMATOGRAPHY (HPLC). SPECIFIC AIM 2: DETERMINE THE EFFICACY OF ?-TOCOTRIENOL AS A COUNTERMEASURE AGAINST MIXED CHARGED PARTICLE RADIATION-INDUCED GENOMIC INSTABILITY. AT THE AGE OF 4 MONTHS A TOTAL OF 96 TRP53?P/?P MICE WILL BE TRANSPORTED TO BNL AND EXPOSED TO RADIATION AT NSRL AS DESCRIBED ABOVE. AFTER RADIATION IS COMPLETE THE MICE WILL BE TRANSPORTED TO THE LABORATORY OF RUPAK PATHAK PHD AT UAMS. AT 1 WEEK AND 4 WEEKS AFTER IRRADIATION MICE WILL BE USED TO ASSESS CHROMOSOMAL ABERRATION IN BONE MARROW CELLS WITH MULTIPLE FLUORESCENCE IN SITU HYBRIDIZATION (MFISH) AND SPECTRAL KARYOTYPING (SKY) USING ROUTINE PROTOCOLS IN THE PATHAK LABORATORY.$965,940
