Abstract

This project examines the clinical presentation and outcomes of rheumatic diseases in various communities within the Washington, D.C. metropolitan area. Through the NIAMS Community Health Clinic, subjects with rheumatic complaints are evaluated, providing clinicians with firsthand experience in understanding the differences in autoimmune disease phenotypes across various communities, as well as the barriers to care. This project has supported research on differences in understanding standardized self-report outcome assessment tools, determining clinically important improvements in rheumatoid arthritis, the use of complementary and alternative medicine in patients with rheumatic symptoms, the acceptability of yoga as a treatment for arthritis, and understanding the research process and informed consent among various communities. Additionally, researchers have investigated interventions to enhance the referral process between primary care providers and subspecialists. Serological studies are currently underway to examine immune abnormalities in patients with rheumatoid arthritis and systemic lupus erythematosus. We are collecting disease activity indices and patient-reported outcome data, including social determinants of health, to gain a deeper understanding of healthcare outcomes in various communities. Additionally, we are analyzing data to understand the cardiovascular outcomes in patients with rheumatic diseases across various communities. The study serves as the primary platform for teaching and training the next generation of scientists, including rheumatology fellows and postbaccalaureate trainees.
