Abstract

The primary focus of this clinical research program is chronic graft-versus-host disease (GVHD), the most serious late complication after allogeneic hematopoietic stem cell transplantation (HSCT). Chronic GVHD is a chronic alloimmune and autoimmune disorder characterized by protean clinical manifestations, immunosuppression, immune dysregulation, decreased organ function, disability and decreased survival of patients. There have been no standard criteria to conduct clinical trials in chronic GVHD. Consequently, little progress has been made in preventing or treating this devastating late effect in survivors of cancer therapy. To address these challenges, we initiated a clinical research program to mobilize the unique strengths of the NIH and ultimately develop effective and safer therapies of chronic GVHD. Specific Aim 1:To utilize the multidisciplinary inter-institute chronic GVHD clinical program at the NIH as a platform for in depth study of the spectrum of clinical phenotypes of chronic GVHD including rare and severe cases, and for developing better chronic GVHD evaluation tools. Specific Aim 2:To study chronic GVHD biology, in a collaborative effort, as a platform for investigating biologically-based clinical classifications, identifying potential new biomarkers or points for therapeutic intervention. Specific Aim 3:To develop new systemic or organ-specific chronic GVHD treatments and drug development pathways. Specific Aim 4: To conduct national and international extramural collaborations for promoting advancements in chronic GVHD research and patient care.
