Abstract

With this fifth competing renewal application, the New York Node (NYN) will build on its existing structure, expertise, and partnerships to support NIDA’s objectives to improve addiction care nationwide through research conducted by its Clinical Trials Network (CTN). Situated in the largest U.S. metropolitan area, the NYN contributes expertise in virtually all key CTN priorities outlined in RFA-DA-25-027, including: capacity to conceptualize, design, lead, and support large trials; collaborations with health systems and community-based substance use disorder (SUD) services providers; and robust experience in developing and testing new therapies and delivery strategies, implementation science, and using existing data sources to explore new research questions. The NYN is led by MPIs McNeely (New York University) and Nunes and Campbell (Columbia University), with deep CTN research experience and complementary expertise. Infrastructure: Includes two additional academic medical centers (Rutgers, Einstein-Montefiore), 7 large health systems, and various types of community organizations and investigators that broaden the scope and improve the science and dissemination of our research. Multi-site clinical trials experience: Since inception, the NYN has led, or is leading, 25 CTN trials and ancillary studies. Accomplishments in the past 5 years include: 1) leading 5 completed multi-site trials; 2) leading 6 studies that will continue into the new funding cycle; 3) contributing sites to 3 other CTN trials; and 4) producing 92 peer-reviewed publications. Research agenda: The NYN works across the translational spectrum to develop and conduct trials of SUD prevention and treatment interventions, using the Consolidated Framework for Implementation Research as an overarching conceptual model to keep adoption and implementation at the forefront of the work. Our six guiding themes for the new grant cycle are to conduct research that seeks to 1) Broaden the reach of SUD services through low-barrier and community-based treatment models; 2) Integrate evidence-based SUD treatment in healthcare settings, including medical settings and community programs; 3) Increase the effectiveness of SUD treatment by addressing mental health comorbidities; 4) Investigate novel treatments, including medications and digital therapeutics; 5) Use existing data sources to study interventions in real-world settings and inform new research questions by leveraging our experience with EHR and administrative data, advanced analytic methods, and a Data and Statistics Group supporting secondary analysis of CTN trials; and 6) Train the next generation of addiction researchers. The NYN proposes to conduct 6 new trials addressing CTN priorities, while also developing studies in partnership with our SUD services partners and providing sites for studies led by other CTN nodes. Through these activities, the NYN advances the mission of the CTN to conduct research with high relevance for improving treatment and prevention practices and ameliorating the detrimental health impacts of substance use, by making evidence-based services more available, accessible, and acceptable to people with SUD.
