Abstract
Abstract The catchment area of Karmanos Cancer Institute (KCI) includes Southeast Michigan and the city of Detroit. According to the 2022 U.S. Census, 78% of Detroiters identify as Black, 32% live in poverty compared to 13% in the state of Michigan, and the majority of census tracts in the city of Detroit are medically underserved areas (MUAs) based on HRSA definitions. Engaging this population in cancer prevention and control efforts is essential. In the proposed work, we will address Track 2 with a focus on the General Population (30+ years) and breast cancer screening. Our preliminary work suggests some delay in mammography screening due to the COVID-19 pandemic, consistent with other work indicating missed or delayed cancer screenings in the first year of the pandemic 1 2 and lower than pre-pandemic screening levels in the second year 3, raising concerns about a future risk in late-stage diagnoses. This is especially concerning given the breast cancer mortality rates of 34.3 per 100,000 among Black women in Detroit compared to 21.3 per 100,00 among White women in the KCI catchment area. Breast cancer screening promotion is consistent with an emerging KCI OCHECE initiative: Karmanos Academy, a program with the primary goal of building community capacity for improving cancer prevention and control throughout the KCI catchment area. Through Karmanos Academy, community members will be trained as lay health advisors (LHAs) to deliver relevant interventions within local organizations and institutions as well as their own social networks. The proposed work has three aims: Aim 1: Adapt an existing LHA-led psychoeducational program to increase mammography adherence among Black women in the metropolitan Detroit area in collaboration with community stakeholders; Aim 2: Collaborate with existing KCI OCHECE community partners to enlist and train community members as LHAs who will deliver the adapted psychoeducational program; Aim 3: Partner with KCI's Marketing and Business Development Department to develop and implement a social media toolkit to promote mammography adherence among Black women in the metropolitan Detroit area. We will create a video-based adaptation of the Real Talk intervention to increase mammography adherence among Black women aged 40 years and older. This video will be an integral part of LHA-led sessions on this topic, session coordinated in tandem with a social media campaign. Ultimately, the proposed work will improve cancer literacy and breast cancer knowledge among Black women in metro Detroit through strategic use of multiple communication channels.
